Nonprofit leaders, health care providers, and community-based organizations are invited to an essential workshop exploring the groundbreaking changes in New York State’s referral and reimbursement processes for Health-Related Social Needs (HRSN). Hosted by the Palisades Institute of Dominican University New York in partnership with Access: Supports for Living, this event will provide actionable insights into how Social Care Networks (SCNs) are reshaping social service delivery and collaboration.

As New York State integrates Social Care Networks to create interoperability among Medicaid managed care organizations (MCOs), health care providers, and community-based organizations, this workshop will be critical for nonprofits and other HRSN providers seeking to understand and leverage these changes.

Bonus Hands-On Training Opportunity
Attendees will also learn about an exclusive hands-on training workshop series offered by Dominican University New York in partnership with the Interoperability Institute, scheduled for February. This training will focus on Fast Healthcare Interoperability Resources (FHIR®)—a foundational technology driving innovation in Social Care Networks and advancing health equity initiatives.
This transformative workshop series is designed to empower nonprofit leaders, community-based organizations, and students with essential skills in healthcare data interoperability. Nonprofits navigating the evolving landscape of healthcare services and data will gain valuable tools and insights through this hands-on learning experience. Participants will explore the latest developments in New York State’s health equity reform, learn how to adapt to the changing technical requirements outlined by the state’s 1115 Waiver, and discover how the FHIR standard can improve care coordination and outcomes for vulnerable populations. Certificates will be issued to those who complete the 15 hour training.
